All contributed data should be derived from research experience associated directly or indirectly with peer-reviewed published work. Previously unpublished contributions based on the experience of active researchers are acceptable, and will be cited as "Personal Communications." If copyright infringement is a possibility, written permission for re-publication, including online use, must be obtained from the author(s) and publisher. A sample permission request letter is provided here for convenience.

Contributors' names and institutional affiliations will appear on the Acknowledgments Page.
